The record must support the next safe decision

A client-information record is a clinical communication and accountability tool, not a private memory aid. Document the relevant assessment evidence, professional interpretation, nutrition problem, plan, education or counseling provided, client response, coordination, monitoring criteria, and follow-up. Make the entry accurate, timely, concise, specific, and understandable to authorized readers. Record what was observed, measured, reported, decided, and done, with sources and uncertainty when they matter. Avoid copied material that was not verified, stigmatizing labels, speculation presented as fact, unexplained abbreviations, and irrelevant personal detail. The test is whether another qualified professional can understand the reasoning, continue care, and identify unresolved safety needs.
